Empowering the Silenced: Trauma-Focused CBT for Psychosis


Session Outline

People with psychosis have high rates of trauma exposure and post-traumatic stress-disorder (PTSD), which is associated with worse clinical outcomes. However, concerns about implementing trauma-focused therapies in psychosis are common, due to concerns about iatrogenic harm. Further, the complexity of psychosis and trauma effects presents a challenge for clinicians in determining how to intervene. This talk will address these issues by providing an overview of the evidence relating to trauma therapies in psychosis.  The therapeutic model and therapy protocol employed in the STAR (Study of Trauma and Recovery) trial (Peters et al., 2025), the largest RCT for trauma therapy in psychosis, will be introduced. Based on theoretical models highlighting the interplay between PTSD and psychosis, the therapy protocol targets evidence-based psychological mechanisms that play a role in both. The talk will be illustrated with case examples and lived experience testimonials.

 

 

Learning objectives

·      Define trauma and describe its relationship to psychosis.

·      Identify the mechanisms that account for the relationship between trauma and psychosis.

·      Analyze the evidence base for trauma-focused therapies in psychosis.

·      Demonstrate how the STAR Trauma-focused Cognitive Behavioural Therapy for Psychosis (Tf-CBTp) therapy protocol can be applied to promote recovery and reduce distressing symptoms in trauma survivors with PTSD and psychosis.

 

About the trainer

Dr Nadine Keen is a Consultant Clinical Psychologist and Clinic Coordinator of the award-winning Psychological Interventions Clinic for Outpatients with Psychosis (PICuP) at South London and Maudsley NHS Trust, London, UK. Nadine has specialised in working with post-traumatic stress and psychosis for the last 20 years and provides Trauma-focused CBT for psychosis (Tf-CBTp) and Eye Movement Desensitisation and Reprocessing (EMDR) therapy. She is passionate about improving therapy provision for trauma survivors with PTSD and psychosis and is a co-applicant and co-therapy lead on a large NIHR funded UK multisite randomised controlled trial: the STAR (Study of Trauma and Recovery) trial which investigated the effectiveness of trauma focused CBTp for people experiencing PTSD and co-morbid psychosis. Nadine delivers frequent national and international workshops and teaching events in this field and runs supervision groups for therapists delivering Tf-CBTp, as well as CBT for psychosis more broadly. Nadine is also involved in other research in this area and supervises a range of doctoral research projects in this field, mainly at the Institute of Psychiatry, Psychology and Neuroscience (IOPPN) at King’s College London (KCL).
